If you are a patient, or a caregiver of a pre-teen, and you want a Sickle Cell advocate (also called a Healthcare Navigator), please fill out the form below. We will contact you and pair you with one. From below, please select “Patient (or Caregiver) Seeking SCD Navigator” so you can register.

 

2)Registering as a patient, but you’re otherwise stable?

You’re stable, and that’s great. Our active-patient registration also covers you, so you can register with us even if you don’t need any assistance right now. By joining, you’ll be eligible to participate in patient- and caregiver-focused events. From below, please select “Patient (or Caregiver) Seeking SCD Navigator so you can register. Once we contact you, you may decline meeting with us or pairing with a Navigator at that time.
